Most recent specification changes (updated 02.08.2023):

1. Identifiers are now persistent for Defacto-issued carrierId, planId, and practitionerId

2. Added extPlanId, extPlanType to Plan to support multiple, externally-issued plan ID types

3. Added Research object to Plan to include Defacto-researched values
